*Electronic submission (of the specified Post-approval Forms) by the Principal Investigator or by the supervisor in case of student research is acceptable. 

 Original signature on the forms is not required when the request form is directly received from the Principal Investigator/Supervisor of student research.

Instructions/Tips:

  1. Be sure to review the Procedures for Completing Requests for Ethics Review guideline that stipulates the SMU REB's requirements involving requests for ethics review.
  2. Be sure you download the most current application form from this website and submit a complete application package.  (Applications submitted using old forms will not be processed.)
  3. A complete student application package includes: a. Form 1B or 1C b. Draft Informed Consent Form and c. all supporting  documentation as relavant to your research. (For 1B: 1 original plus 3 copies, For 1C: 1 original copy).
  4. A complete faculty application package includes: a. Form 1A or 1C, b. Draft Informed Consent Form and c.  all supporting documentation as relavant to your research. (For  1A: 1 original plus 3 copies, For 1C: 1 original copy.)
  5. Please see the 'Appliction Form Checklist' document to determine what constitutes a "supporting document" or call the Office of Research Ethics for guidance. (Generally, supporting documents are interview protocols, surveys, questionnaires, scales, scripts, draft interview questions, invitation letters, letters of support, feedback forms, debriefing forms, advertisements, other research tools, etc.)
  6. You only need to submit one copy for: Modification requests (Form 2), Annual Renewall requests (Form 3), Annual Course Reports (Form 4) and Completion of Research forms (Form 5).
